Complete Intake Assessment

To receive our outpatient behavioral services, you’ll need to fill out and submit an intake assessment form. Assessments are offered via telehealth or during our walk-in hours: Monday-Friday, 8 am – 4 pm.

A licensed clinical therapist will be assisting you with completing your intake assessment. Please note: the intake assessment is only to confirm admission to our clinic. It is not mean the receipt medication right away.

Things to bring with you to your visit:

• Proof of identification and/or residence and your health insurance card
• A list of all current medications (and side effects)
• If you have a referral, please bring your copy with you.
